These Terms of Use govern your use of leveldose.com and the services offered by Leveldose LLC. By using the site, you agree to these Terms. If you do not agree, do not use the service.

  • What Level Dose is — and isn't. Level Dose is a technology platform that connects you with independent, licensed healthcare providers (employed by or affiliated with our medical group partners) for telehealth services. Level Dose itself does not practice medicine. Providers exercise independent clinical judgment, and a provider may determine that the service or a requested treatment is not appropriate for you.
  • Not for emergencies. The service is not for medical emergencies. If you are experiencing an emergency, call 911 or go to the nearest emergency room.
  • Eligibility. You must be 18 or older, a resident of a state we serve (all U.S. states except Arizona and Hawaii), and able to form a binding contract.
  • Your account. You agree to provide accurate, complete information and to keep your credentials secure. You are responsible for activity under your account. Providing false health information may affect your safety and your eligibility for care.
  • Payments and subscriptions. You authorize us (and our payment processor) to charge your payment method for the fees and recurring charges you select. Prices, what each price includes, and renewal terms are disclosed at checkout. You can cancel as described in the Returns & Refund Policy.
  • Intellectual property. All site content, branding, and software are owned by Leveldose LLC or its licensors and may not be copied or used without permission.
  • Acceptable use. You agree not to misuse the service, attempt to access it unlawfully, or interfere with its operation.
  • Disclaimers. The service is provided "as is." We do not warrant uninterrupted or error-free operation. Provider services are subject to the separate Telehealth Consent and to each provider's professional judgment.
  • Limitation of liability. To the maximum extent permitted by law, Leveldose LLC's aggregate liability arising out of the service is limited as set out here. (Counsel will calibrate this clause and the carve-outs to your jurisdictions.)
  • Dispute resolution. (Arbitration and governing-law provisions are highly state-specific and consequential — leave these for counsel to draft. Do not paste a generic arbitration clause.)
  • Changes. We may update these Terms; continued use after an update means you accept the revised Terms.
