This Privacy Policy explains how Leveldose LLC ("Level Dose," "we," "us") collects, uses, and shares information when you visit leveldose.com or use our services. Health information you share in the course of receiving care is also governed by our HIPAA Notice of Privacy Practices and by applicable state and federal health privacy law; where those rules apply, they control.

Information we collect

  • Information you provide: name, contact details, date of birth, account credentials, payment information, and the health, medical history, and lifestyle information you submit during intake and consultations.
  • Information collected automatically: device and browser data, IP address, pages viewed, and similar analytics data collected through cookies and comparable technologies.
  • Information from third parties: your affiliated provider(s), pharmacies, laboratories, and payment processors, as needed to deliver care and process orders.

How we use information

To verify identity and eligibility; to connect you with licensed providers; to process consultations, prescriptions, orders, and payments; to communicate with you about your care and account; to operate, secure, and improve our services; and to comply with legal obligations.

How we share information

We share information with the providers and our medical group partners involved in your care; with pharmacies and laboratories that fulfill orders; with service providers acting on our behalf under contract (e.g., hosting, payments, analytics); and where required by law.

We do not sell your personal information.

We do not use or disclose your protected health information for marketing without your authorization. See the CCPA/CPRA section for choices regarding "sharing" for cross-context behavioral advertising.

Cookies and tracking

We use cookies and similar technologies for essential functionality, analytics, and (where permitted) advertising. You can control non-essential cookies through your browser settings and our cookie preferences tool.

Note: we do not use third-party advertising trackers on pages where you submit health information.

Data security and retention

We maintain administrative, technical, and physical safeguards designed to protect your information, and retain it as long as needed to provide services and meet legal, medical-records, and regulatory requirements.

Our services are not directed to and not available to anyone under 18.
