Telehealth Consent

Effective: June 1, 2026

By using Level Dose, you consent to receive healthcare services via telehealth. Please read this carefully before proceeding.

  • What telehealth is. Telehealth uses secure electronic communication — including online questionnaires, messaging, and where applicable audio or video — to deliver care without an in-person visit. Your provider will use the information you submit to evaluate you and determine appropriate care.
  • Your honesty matters. The quality and safety of your care depend on the accuracy and completeness of the information you provide. Incomplete or inaccurate information can lead to an inappropriate treatment decision.
  • Benefits. Improved access and convenience, and the ability to receive care from licensed providers without travel.
  • Risks and limitations. Telehealth has limits. Without an in-person exam, a provider may not detect a condition that an in-person visit would. Technology can fail or delay communication. In some cases, your provider may determine that telehealth is not appropriate and may recommend in-person care. There is no guarantee that you will be prescribed any particular medication or treatment; that decision rests solely with your provider's clinical judgment.
  • Not for emergencies. Telehealth is never for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
  • Privacy. Your information is handled in accordance with our HIPAA Notice of Privacy Practices and Privacy Policy. No electronic system is perfectly secure, but we use safeguards designed to protect your information.
  • Your consent. By proceeding, you acknowledge that you have read and understood this consent, that you have had the opportunity to ask questions, and that you voluntarily consent to receive care via telehealth. You may withdraw this consent at any time, though it will not affect care already provided.

(State-specific telehealth consent requirements vary — several states require particular language or a separate acknowledgment. Have counsel confirm requirements for each state in all U.S. states except Arizona and Hawaii.)
