HIPAA Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Who this covers

This Notice applies to the providers of our medical group partners and to Leveldose LLC to the extent it handles your protected health information ("PHI") in connection with your care.

How we may use and disclose your PHI without your authorization

• Treatment — to provide and coordinate your care among providers, pharmacies, and labs.
• Payment — to bill and obtain payment for services.
• Health care operations — for quality, training, and administrative functions.
• As required or permitted by law — including for public health activities, to avert a serious threat to health or safety, and for certain law enforcement or legal processes where the law permits.

Uses and disclosures that require your written authorization

Most uses for marketing, any sale of PHI, and most disclosures of psychotherapy notes. You may revoke an authorization in writing at any time.

Your rights regarding your PHI

• Request access to and a copy of your records.
• Request corrections/amendments.
• Request an accounting of certain disclosures.
• Request restrictions on certain uses and disclosures.
• Request confidential communications (e.g., a specific contact method).
• Receive a paper copy of this Notice, even if you agreed to receive it electronically.
• Be notified following a breach of unsecured PHI.

Our duties

We are required by law to maintain the privacy of your PHI, to provide you this Notice, to follow its current terms, and to notify you of breaches as required.

Complaints

You may file a complaint with us at support@leveldose.com or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

Changes

We may change this Notice and apply the revised terms to all PHI we maintain; the current version will always be posted at leveldose.com.
